Sertralin ist ein Antidepressivum. Es ist nirgends gegen vorzeitigen Samenerguss zugelassen und steht in dieser Kategorie, weil Ärzte in Ländern ohne Dapoxetin es dafür off-label verordnen. Die Fachinformation unten ist die für Depression und Angststörungen, denn eine andere gibt es nicht.

Zoloft

Zugelassene Anwendungsgebiete
  • Premenstrual dysphoric disorder (PMDD) ZOLOFT is a selective serotonin reuptake inhibitor (SSRI) indicated for the treatment of:

Darf nicht angewendet werden bei
  • Concomitant use of monoamine oxidase inhibitors (MAOIs), or use within 14 days of stopping MAOIs
  • Concomitant use of pimozide
  • Known hypersensitivity to sertraline or excipients
  • ZOLOFT oral solution only: Concomitant use of disulfiram
Wechselwirkungen
  • Protein-bound drugs: Monitor for adverse reactions and reduce dosage of ZOLOFT or other protein-bound drugs (e.g., warfarin) as warranted.
  • CYP2D6 substrates: Reduce dosage of drugs metabolized by CYP2D6. Clinically Significant Drug Interactions Table 5 includes clinically significant drug interactions with ZOLOFT. Table 5. Clinically-Significant Drug Interactions with ZOLOFT Monoamine Oxidase Inhibitors (MAOIs) Clinical Impact: The concomitant use of SSRIs including ZOLOFT and MAOIs increases the risk of serotonin syndrome.
Warnhinweise und Vorsichtsmaßnahmen
  • Serotonin Syndrome: Increased risk when co-administered with other serotonergic agents, but also when taken alone. If it occurs, discontinue ZOLOFT and serotonergic agents and initiate supportive treatment.
  • Increased Risk of Bleeding: Concomitant use of aspirin, nonsteroidal anti-inflammatory drugs (NSAIDs), other antiplatelet drugs, warfarin, and other anticoagulants may increase this risk.
  • Activation of Mania/Hypomania: Screen patients for bipolar disorder.
  • Seizures: Use with caution in patients with seizure disorders.
  • Angle Closure Glaucoma: Avoid use of antidepressants, including ZOLOFT, in patients with untreated anatomically narrow angles.
  • QTc Prolongation: ZOLOFT should be used with caution in patients with risk factors for QTc prolongation.
  • Sexual Dysfunction: ZOLOFT may cause symptoms of sexual dysfunction. Häufige Nebenwirkungen
  • most common adverse reactions (≥5% and twice placebo) in all pooled placebo-controlled clinical trials of all ZOLOFT-treated patients with MDD, OCD, PD, PTSD, SAD and PMDD were nausea, diarrhea/loose stool, tremor, dyspepsia, decreased appetite, hyperhidrosis, ejaculation failure, and decreased libido (see Table 3).

Pregnancy Exposure Registry There is a pregnancy exposure registry that monitors pregnancy outcomes in women exposed to antidepressants during pregnancy. Wie es wirkt

Sertralin erhöht die zwischen Nervenzellen verfügbare Serotoninmenge, worauf seine zugelassene Anwendung bei Depression und Angststörungen beruht. Ein verzögerter Samenerguss ist eine gut dokumentierte Folge derselben Wirkung und in der Fachinformation unter den Nebenwirkungen aufgeführt. Die Off-label-Verordnung nutzt diese Nebenwirkung gezielt.

Original und Generikum

Seit Jahrzehnten patentfrei und eines der günstigsten verschreibungspflichtigen Mittel überhaupt. Der Preis ist nicht der Grund, warum es seltener eingesetzt wird als Dapoxetin.

Was es kostet

Günstig, und in den meisten europäischen Gesundheitssystemen für die zugelassenen Indikationen erstattet. Die Erstattung einer Off-label-Anwendung ist eine eigene Frage mit je Land anderer Antwort.

Wie man es legal bekommt

Verschreibungspflichtig. Hier zählt ein echtes Gespräch mit der verordnenden Person mehr als sonst, weil das Mittel außerhalb seiner Zulassung eingesetzt wird und weil das Absetzen eines SSRI etwas anderes ist als das Weglassen einer Tablette vor dem Sex.

Fragen

Ist eine Off-label-Verordnung zulässig?
Ja. Eine Ärztin oder ein Arzt darf ein zugelassenes Mittel für eine von der Behörde nicht bewertete Anwendung verordnen und übernimmt dafür die Verantwortung. Eine Zulassung ist das nicht, und diese Seite stellt es auch nicht so dar.
Warum handelt die Fachinformation von Depression?
Weil das Mittel dafür zugelassen ist. Für diese Anwendung gibt es keine Fachinformation, und die echte zu zeigen ist die ehrliche Art, das darzustellen.
Nimmt man es täglich oder vor dem Sex?
Die zugelassene Fachinformation beschreibt die tägliche Einnahme für die zugelassenen Erkrankungen. Wie es off-label eingesetzt wird, entscheidet die verordnende Person, und eine Nachschlageseite sollte das nicht vorgeben.
