- Formes et dosages
- VIAGRA is supplied as blue, rounded diamond shaped film-coated tablets containing sildenafil citrate equivalent to 25 mg, 50 mg, or 100 mg of sildenafil.
- Tablets are debossed with "VIAGRA" on one side and "VGR 25", "VGR 50" or "VGR 100" on the other to indicate the dosage strengths.

- Mises en garde et précautions
- Patients should not use VIAGRA if sexual activity is inadvisable due to cardiovascular status
- Patients should seek emergency treatment if an erection lasts >4 hours. Use VIAGRA with caution in patients predisposed to priapism
- Patients should stop VIAGRA and seek medical care if a sudden loss of vision occurs in one or both eyes, which could be a sign of non arteritic anterior ischemic optic neuropathy (NAION). VIAGRA should be used with caution, and only when the anticipated benefits outweigh the risks, in patients with a history of NAION. Patients with a "crowded" optic disc may also be at an increased risk of NAION.
- Patients should stop VIAGRA and seek prompt medical attention in the event of sudden decrease or loss of hearing
- Caution is advised when VIAGRA is co-administered with alpha-blockers or anti-hypertensives. Concomitant use may lead to hypotension

- VIAGRA is not indicated for use in females. There are no data with the use of VIAGRA in pregnant women to inform any drug-associated risks for adverse developmental outcomes. Animal reproduction studies conducted with sildenafil did not show adverse developmental outcomes when administered during organogenesis in rats and rabbits at oral doses up to 16 and 32 times, respectively, the maximum recommended human dose (MRHD) of 100 mg/day on a mg/m 2 basis (see Data).

- The physiologic mechanism of erection of the penis involves release of nitric oxide (NO) in the corpus cavernosum during sexual stimulation. NO then activates the enzyme guanylate cyclase, which results in increased levels of cyclic guanosine monophosphate (cGMP), producing smooth muscle relaxation in the corpus cavernosum and allowing inflow of blood. Sildenafil enhances the effect of NO by inhibiting phosphodiesterase type 5 (PDE5), which is responsible for degradation of cGMP in the corpus cavernosum.

Comment il agit
Le sildénafil bloque une enzyme, la phosphodiestérase de type 5, celle qui met normalement fin à l'érection en dégradant la molécule-signal qui maintient les vaisseaux du pénis relâchés. En la bloquant, ce signal dure plus longtemps. Or ce signal n'est produit que pendant l'excitation sexuelle : c'est pourquoi la notice précise que sans stimulation, rien ne se produit. Le médicament prolonge une réponse, il ne la crée pas.
Princeps et générique
Le brevet a expiré en Europe en 2013 et aux États-Unis en 2017. Le sildénafil générique est fabriqué par des dizaines de laboratoires et contient la même substance active aux mêmes dosages que l'original, contrôlée selon la même norme. Au Royaume-Uni, une version à 50 mg est vendue en pharmacie sans ordonnance après entretien avec le pharmacien, sous le nom de Viagra Connect : un statut juridique différent de celui de la version sur ordonnance.
Ce qu'il coûte
C'est cet effondrement des prix qui rend la catégorie intéressante. Le produit de marque se vend toujours à plusieurs fois le prix du générique, tandis que le sildénafil générique en pharmacie européenne revient à une fraction de ce montant par comprimé, selon le dosage, la taille de la boîte et le pays. La substance active est identique : la différence tient entièrement à la marque et à l'emballage.
Comment l'obtenir légalement
Sur ordonnance dans l'Union européenne, et sur ordonnance à tous les dosages aux États-Unis. Le Royaume-Uni fait exception : la version pharmacie à 50 mg s'y achète après un échange avec le pharmacien. Ailleurs, la voie légale sans rendez-vous au cabinet passe par une clinique en ligne où un prescripteur examine un questionnaire, et les questions sur les traitements cardiaques en sont l'objet même, pas une formalité.

- Pediatric Patients (1 to 17 Years Old) REVATIO is indicated in pediatric patients 1 to 17 years old for the treatment of pulmonary arterial hypertension (PAH) (WHO Group I) to improve exercise ability and, in pediatric patients too young to perform standardized exercise testing, pulmonary hemodynamics thought to underlie improvements in exercise.
- Adults REVATIO is a phosphodiesterase-5 (PDE-5) inhibitor indicated for the treatment of pulmonary arterial hypertension (PAH) (World Health Organization [WHO] Group I) in adults to improve exercise ability and delay clinical worsening.

- For oral suspension: 10 mg/mL (when reconstituted)
- Injection: 10 mg/12.5 mL (0.8 mg/mL) in a single-dose vial

- Vasodilation effects may be more common in patients with hypotension or on antihypertensive therapy.
- Use in pulmonary veno-occlusive disease (PVOD) may cause pulmonary edema and is not recommended.
- Hearing or visual impairment: Seek medical attention if sudden decrease or loss of vision or hearing occurs.
- Pulmonary hypertension (PH) secondary to sickle cell disease: REVATIO may cause serious vaso-occlusive crises. Hypotension REVATIO has vasodilatory properties, resulting in mild and transient decreases in blood pressure.

- Grossesse
- Limited published data from randomized controlled trials, case-controlled trials, and case series do not report a clear association with sildenafil and major birth defects, miscarriage, or adverse maternal or fetal outcomes when sildenafil is used during pregnancy. There are risks to the mother and fetus from untreated pulmonary arterial hypertension (see Clinical Considerations).

- Sildenafil is an inhibitor of cGMP specific PDE-5 in the smooth muscle of the pulmonary vasculature, where PDE-5 is responsible for degradation of cGMP. Sildenafil, therefore, increases cGMP within pulmonary vascular smooth muscle cells resulting in relaxation. In patients with PAH, this can lead to vasodilation of the pulmonary vascular bed and, to a lesser degree, vasodilatation in the systemic circulation.
